Informational Workshop
Applicants are invited to attend an informational session scheduled for February 12, 2013 (3PM to 5PM) at Mission Center Building, 1855 Folsom Street, Room 126.
This workshop will clearly describee the program, service learning, explained the application process, and answered questions. The informational workshop wass not mandatory but strongly encouraged, Please contact us at ucpgrants@ucsf.edu for more information.

Submission Instructions
Grant proposals will require electronic submission of a cover sheet and full proposal to ucpgrants@ucsf.edu. In the event that there is a community partner without electronic submission capabilities, the university partner should serve in this role and submit the application. All proposal elements should be sent in one file in either PDF or MS Word (.doc or .docx) format (additional guidelines listed in the request for proposals). The file should be named in the following format: UCP_2013 Service Learning Grant Application_Project Name (your specific project should be named where it says project name).

Primary community partners who have not received UCP Grants before should also complete and submit a W9 form online. Click here to complete the W9 form. Completion and submission of a W9 will expedite our ability to transfer grant funds to community partners.
